For new patients, please complete and print out our patient registration forms below. Bring them with you to your first visit along with your insurance card and a list of any current medications you are taking. This will help expidite your first visit with us.

For your convenience, you may also submit your new patient information entirely online through our Patient Portal. Contact a member of our staff to register your email address, and you will receive an invitation to complete your registration for the Patient Portal.
At Arizona Center for Digestive Health, we are happy to work with you in caring for your patient’s digestive health needs. Please choose whichever method of referral that is most convenient to you:

Insurance and Billing
Please contact our office directly for the most up to date information regarding our currently accepted insurance carriers. However, due to the rising number of new plans under each carrier, we highly suggest that you contact your insurance carrier to confirm your plan is contracted with our office.
Our office currently accepts Visa, Discover, MasterCard, Cash and Checks as a form of payment. Please be prepared to make all copays at the time of service.
